E2E CONSULTING LLC SMS AND MMS VERBAL CONSENT SCRIPT Script version: 2026-11-03-v2 Planned initial enrollment procedure for phone calls and in-person conversations. This document is not an enrollment form. Reading it or contacting E2E does not subscribe anyone. Staff will use the procedure when the messaging service is available and its confirmation, HELP, and STOP handling have been verified. CONSENT REQUEST An E2E representative will read the following before enrolling a recipient: "E2E Consulting would like to send you text and picture messages about your inquiry, support requests, and project updates. These will not be marketing broadcasts. Sending us photos or attachments will be optional. Message frequency varies. Message and data rates may apply. You can reply STOP to unsubscribe or HELP for assistance. You can also get help through our contact form at e2e.consulting/contact. If you unsubscribe, we will not resume messages unless you give fresh express written consent. Verbal consent alone will not restore enrollment. Agreeing is voluntary and is not a condition of purchasing services. Our Terms and Conditions are at e2e.consulting/terms, and our Privacy Policy is at e2e.consulting/privacy. Do you agree to receive these SMS and MMS messages from E2E Consulting at the mobile phone number you provide? Please answer yes or no." IF THE RECIPIENT SAYS YES Before using this initial enrollment procedure, staff will check whether the recipient previously opted out. If so, the written re-consent rule below will apply instead; a new verbal yes will not override the opt-out. The representative will confirm the mobile number, record the affirmative consent, and say: "Thank you. We will record your permission and send you a text confirming your enrollment. You can reply STOP at any time to unsubscribe." Only an explicit affirmative response will authorize enrollment. Silence, providing a phone number, agreeing to general terms, or submitting a website inquiry will not count as consent. Staff will record the number, date and time, phone or in-person method, agreed purpose, representative, and script version in a private consent record before initiating the confirmation message. The service will send this confirmation promptly after consent: "E2E Consulting: You are enrolled for SMS/MMS about your inquiry, support requests, and project updates. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for assistance. Help: https://e2e.consulting/contact Terms: https://e2e.consulting/terms Privacy: https://e2e.consulting/privacy" IF THE RECIPIENT SAYS NO OR DOES NOT CLEARLY AGREE The representative will not enroll the recipient or send an enrollment text. The representative will say: "No problem. We will not enroll you in text messaging. We can continue our conversation by phone or email." WITHDRAWAL AND HELP Recipients will be able to reply STOP to unsubscribe and HELP for assistance. Staff will honor other clear withdrawal requests as well. An opt-out will not be bypassed by changing sending numbers. Enrollment will not be resumed without fresh express written consent. Verbal consent alone will not restore enrollment. Staff will retain the recipient's informed written re-consent before resuming program messages. If that evidence cannot be verified, the recipient will remain opted out, even if the provider has cleared its block list. Staff will not send a text asking an opted-out recipient to re-enroll. A recipient may receive one final opt-out confirmation. Public policies: https://e2e.consulting/privacy https://e2e.consulting/terms Support and privacy requests: https://e2e.consulting/contact
